Gulf Coast Regional Blood Center and Continental BusinessFirst

  1. GIVEAWAY DETAILS. Gulf Coast Regional Blood Center and Continental Airlines, the official Commit for Life airline, want you to save lives and see the world! Commit for Life members who donate or attempt to donate Sept. 1 through Nov. 30 will be entered to win one of four pairs of Continental Airlines tickets for domestic travel or one of two pairs of Continental BusinessFirst tickets to visit select international destinations. In addition, gold-level Commit for Life donors will automatically receive an extra entry for donating or attempting to donate in September, October and November!
  2. ENTRY. No purchase or donation necessary. Entry can be made by (i) becoming Gulf Coast Regional Blood Center Commit for Life member and donating or attempting to donate at any Gulf Coast Regional Blood Center blood drive or Neighborhood Donor Center between Sept. 1, 2010, and Nov. 30, 2010 (the "Giveaway Period"), or (ii) being present at any Gulf Coast Regional Blood Center blood drive or Neighborhood Donor Center during the Giveaway Period, receiving information regarding becoming a Gulf Coast Regional Blood Center Commit for Life member and agreeing to be contacted regarding becoming a Gulf Coast Regional Blood Center Commit for Life member and donating. Individuals must be a Gulf Coast Regional Blood Center Commit for Life member to participate. Autologous, direct and therapeutic procedures do not automatically qualify the donor for entry; however, any such donor may enter pursuant to method (ii) described in the foregoing sentence. Each donation or attempt at donation from a Commit for Life member will entitle the donor to one automatic entry, and each receipt of information regarding becoming a Gulf Coast Regional Blood Commit for Life member will entitle the recipient to one entry instruction form, provided that no participant may be entitled to more than one entry form. Entry also can be made by mailing a self-addressed, stamped envelope to Gulf Coast Regional Blood Center, c/o Commit for Life Continental BusinessFirst, 1400 La Concha Lane, Houston, Texas, 77054. Entrant will then receive one entry form, as well as information regarding becoming a Gulf Coast Regional Blood Center Commit for Life member and agreeing to be contacted regarding becoming a Gulf Coast Regional Blood Center Commit for Life member and donating.  Returned information must be accurately completed to be eligible, which includes first and last name, phone number, address, city, state, zip code, e-mail address and date of birth.  Incomplete entry forms will be invalid. All forms must be returned to Gulf Coast Regional Blood Center and in hand by end of business on Nov. 30 to be included.
  3. ELIGIBILITY. The giveaway is open to Gulf Coast Regional Blood Center Commit for Life members who are legal residents, 17 years or older, of the State of Texas. Employees and immediate family members of the Gulf Coast Regional Blood Center and Continental Airlines are not eligible. To be eligible entrants must (i) have donated or attempted to donate with Gulf Coast Regional Blood Center during the Giveaway Period, be a Gulf Coast Regional Blood Center Commit for Life member or (ii) have received information during the Giveaway Period regarding becoming a Gulf Coast Regional Blood Center Commit for Life member and agreed to be contacted by Gulf Coast Regional Blood Center regarding donating, and entering via entry form.  If the winner of the giveaway is a minor, Continental shall deliver the prize to one of the minor's parents or the minor's guardian as custodian for the minor under the Texas Uniform Transfers to Minors Act and any such delivery shall be a full discharge of Gulf Coast Regional Blood Center with respect thereto. The minor shall complete the entry form with information regarding the minor's parent or guardian who will act as custodian as set forth in Paragraph 1.   Gulf Coast Regional Blood Center gold-level Commit for Life donors will automatically receive an extra entry for donating or attempting to donate in September, October and November.
  4. PRIZE. Two (2) Continental Airlines tickets to travel to any destination in the contiguous United States where Continental flies, or two (2) Continental BusinessFirst tickets to travel to any Transatlantic, Pacific, Hawaii, South America, Europe, India, Guam or Asia destination where Continental flies.  Approximate retail value of Continental BusinessFirst tickets is not disclosed.
  5. TAXES AND EXPENSES. The winner is required to pay federal income tax on the value of the prize in your tax bracket.  In addition, the winner is required to cooperate with Gulf Coast Regional Blood Center and Continental Airlines in providing information to prepare any additional regulatory filings, including, but not limited to, IRS Form 1099. In the event that the winner is unable or declines to pay the federal income tax, the winner shall be disqualified, and a new winner will be selected randomly from the remaining entrants.  The winner of that drawing will have seven (7) days upon notification of winning to meet the requirements set forth herein.  If that winner is unable to meet such requirements, additional winners will be drawn in seven-day intervals, from the remaining entrants, until such time as the winner is able to meet the requirements set forth herein.  Travel insurance is not included in the prize and is winner's responsibility, if desired.   Winner will solely be responsible for any customs duties (where applicable), and any other costs and expenses associated with accepting and using the prize. The winner is responsible for all additional charges that may apply in terms of cancellation, flight changes etc. When additional fees and/or taxes apply, the winner is asked to pay for them at the time of ticketing with a credit card.  No prize substitution shall be permitted except by Continental Airlines who reserves right in its sole discretion to substitute a prize of comparable or greater value.  Prize cannot be sold and/or traded for cash as required by the Food and Drug Administration (FDA).
  6. RANDOM DRAWING. Sponsor will randomly select six winners during the Giveaway Period. Two winners will each receive two BusinessFirst e-tickets, and four winners will each receive two Continental Airlines tickets for domestic travel, as described in section 4. At the conclusion of the Giveaway Period, Gulf Coast Regional Blood Center will utilize the Web site www.graphpad.com/quickcalcs/randomN1.cfm to help select the winner. Odds of winning depend upon the number of eligible entries received. Further details about random drawing can be provided upon request.
  7. PRIZE AWARD. Each winner will be notified by phone and will be required to accept the tickets at 1400 La Concha in the presence of Gulf Coast Regional Blood Center staff. The winner must claim their prize within 14 days of date called. The winner will be required to sign a Affidavit of Eligibility Release, Eligibility Letter, as well as a Model Release, which is for permission to use his or her name in media releases, on the Web site and other media publications.  Failure to claim the prize in a timely manner may cause the prize to be forfeited and awarded to an alternate. Each winner will be required to provide a valid driver's license upon being notified of winner status. Subject to meeting the requirements set forth in Paragraph 4, the winner must take delivery of the prize within 14 days of delivery of notice of availability or the prize may be forfeited and awarded to an alternate. Prize is non-transferable and has no cash value.
  8. GENERAL. By participating, entrants agree (a) to the Official Rules and the decisions of the Sponsor and its agents in all matters relating to the giveaway, which decisions shall be final in all respects; (b) to release and to hold harmless Sponsor, and its subsidiary and affiliated companies and their respective directors, officers and employees from any and all liability, damages, injuries and claims arising in whole or in part from their participation and the acceptance and use of the prize; and (c) that the foregoing parties have neither made nor are in any manner responsible or liable for any warranty, representation or guaranty, expressed or implied, in fact or in law, relative to the prize including but not limited to its quality, mechanical condition or fitness for use.
  9. DISCLAIMER. Sponsor and its agents are not responsible for error, omission, interruption, deletion, defect, delay in operations or transmission, theft or destruction or unauthorized access to or alterations of entry materials, or for technical, network, telephone, computer, hardware or software malfunctions of any kind, or inaccurate transmission of, or failure of Sponsor to receive entry information on account of technical or any combination thereof. If the giveaway becomes technically corrupted or compromised due to any of the foregoing or other factors or non-authorized human intervention, Sponsor reserves the right, in its sole discretion, to (a) terminate the giveaway; and/or (b) select the winner from among all eligible entries received prior to the termination. Notice of any termination will be posted on www.giveblood.org.
  10. WINNER. For name of winner, available after Dec. 14, 2010, mail a self-addressed stamped envelope to: Gulf Coast Regional Blood Center, attention Continental giveaway, 1400 La Concha Lane, Houston, Texas, 77054. Name of winner also will be posted on www.giveblood.org.
